The test note
TEST ONLY, NOT A REAL EXAM. DISCARD THIS NOTE. ***
Mental status: Patient cooperative, no limitations. Awake and alert. Disoriented to time. Attentive during the interview. Non-fluent, effortful speech, phonemic paraphasias. Names 3 of 6 common objects by sight. Follows simple but not complex commands, repeats words and sentences accurately. Mild-moderate dysarthria, intelligible. No neglect.
Cranial nerves: Incomplete homonymous hemianopia to the right. Pupils equal, round, reactive to light. Full versions and ductions, no diplopia. Gaze preference to the left, overcome by oculocephalic maneuver. No nystagmus. Touch and pinprick intact V1-V3 bilaterally. Lower facial weakness on the right, forehead relatively spared. Palate elevates symmetrically, uvula midline. Sternocleidomastoid and trapezius strength 5/5 bilaterally. Tongue midline, no atrophy or fasciculations.
Motor: Normal tone in all extremities. Normal bulk, no fasciculations. No involuntary movements. Right arm drifts to bed, right leg drifts, does not hit bed. Slow, low-amplitude finger taps on the right. Weakness pattern: face = arm > leg.

Reflexes: No Hoffmann sign and no clonus.

Coordination and gait: Not assessable in the right limbs due to weakness. Sits unsupported without truncal ataxia. Gait not tested, fall risk.
